Referral Information

You can access a program at the Counselling & Support Center through one of the following options:

1. Walk in to the Counselling & Support Center at 4838 Lazelle Ave and complete a short, self referral for yourself (with or without assistance).

2. Phone (250)635-2373 and the program assistant will complete a short, self-referral form with you over the phone.

3. Visit the appropriate program page on our website (from the Counselling dropdown menu at the top of the page) and click the “Request More Information” button. You will be asked a few questions in the Information Request Form that will help us determine which program you want, and a support person will contact you.

4. Email intake@ksansociety.ca and request either a password-protected self referral form (for yourself) or a password-protected agency referral form (if you are referring someone). You will have access to an Adobe fillable form. We ask that you email the completed referral back to intake@ksansociety.ca or fax to 250.635.2315.
